Arizona: Implementation of Integrated Services, supported by grant #
6H29MC00005 from the Maternal and Child Health Bureau (MCHB), Title V, Social
Security Act.

SWI Mission: to contribute to the emergent knowledge-base and
disseminate results in ways that are responsive to the beliefs, values and
needs of families and children and the professionals who serve them.
|
SWI conducts research, demonstration, implementation, evaluation and training
projects that advance knowledge, change practice, and thereby improve the
quality of health, education and social services for families, children and
youth with special needs.

Building Community Health in Arizona (BCHA): Statewide Implementation of
Integrated Services
|
Five Parent-led Community Action Teams and the SWI Statewide Steering Committee
address goals that:

establish enrollment/ eligibility process across multiple agencies
-
develop a system for sharing secured, confidential child data
-
maximize resources through models of braided or blended funding
-
streamline care coordination across agencies and sites
